Michael Auld 2008
On 17 January 2008 Mr Auld was working underground as a member of a three person service crew at Cannington Mine preparing to start work on an extension of air services. He was standing at the rear of a Toyota utility with his back to an approach loader operated by another crew member. The plan was to transfer tools and other equipment into the basket on the loader to gain access to the roof of the drive where the existing air line finished.
A third crew member was directing the loader operator forward. However, on signalling to stop, the loader continued and crushed Mr Auld between the loader.
